Adam Chalmers, of Falkirk, formerly Dunblane, passed away suddenly aged 36 years.
The much-loved son of Susan and Neil, brother to Claire and brother-in-law to Daniel and proud uncle (Tabby) to Edie and Stanley and loving partner to Nicola.
The funeral service will be held on Wednesday 10th June, at 3.15pm, in Falkirk Crematorium.
All family and friends are welcome.
The family have asked you wear something that reminds you of Adam, a football shirt, rugby, or band t-shirt would be encouraged.
